Parental Notarized Consent for Tattoos & Piercings on Minors

Notarized parental consent is the process of which you and your parent will visit a local Florida Notary and sign an affidavit that your parent/legal guardian gives you permission to get tattooed or pierced while you are under the age of 18. 

In Florida, you must be at least 16 or 17 years old to get a tattoo with notarized parental consent accompanied by your parent/legal guardian during the procedure and all parties MUST have GOVERNMENT ISSUED ID. Nobody UNDER the age of 16 is legally allowed to get tattooed.

In Florida, you can be any age to get pierced! Minors must have notarized parental consent accompanied by your parent/legal guardian during the procedure and all parties MUST have GOVERNMENT ISSUED ID. 

You will have to bring:

1. Minor that is getting procedure as well as their GOVERNMENT ISSUED ID.
2. Parent must be present to sign, parent must also bring GOVERNMENT ISSUED ID.
3. It helps to bring a birth certificate if there is any name discretion.
4. If you are a legal guardian, you will need to bring copies of legal paperwork that certifies you are. Unfortunately, friends and siblings over 18 do not constitute as legal guardians unless declared by the courts with paperwork proof. Sorry.
5. We know this is a lot, we are sorry, but this is what we need to stay compliant with FL Dept of Health and their codes, and local laws.
